The following forms are official documents approved by TDLR.

Corresponding forms MUST be provided to anyone that may request them at all licensed Vehicle Storage Facilities.

This form is used by insurance companies, tow companies, and companies exempt from licensing through Unified Carrier Registration (UCR) or other exemptions to pick up vehicles from a vehicle storage facility. 

When a vehicle owner or the owner’s authorized representative (Immediate Family Member) presents this form filled out, presents payment, and presents a valid state or federally issued ID, the vehicle storage facility must release the vehicle.  Must also be filled out if they are retrieving personal property from vehicle.
